We are looking for a casual RN on our hospice team. You must be willing to work at least one weekend a month doing visits/admissions for the Metro Hospice program.
The Casual RN plans, organizes, coordinates and provides skilled nursing care to hospice patients according to a plan of care, including advanced nursing interventions related to the patients needs.
Planning: Ongoing development and implementation of the plan of care.
Maintains an individualized plan of care, related to the patients needs, in collaboration with the physician, patient and significant others. Determines appropriate utilization of resources based on clinical assessment, HHRG guideline or Hospice guidelines and standards of care.
Continually reassess the patients health status, environment, and support system with ongoing comprehensive assessment. Plans maximize independence of patient/caregiver.
Evaluates the patients progress and response to care/services delivered and adjusts the plan accordingly. Continually evaluates whether the patients: 1) clinical needs can be met at home 2) are able to care for themselves or have a paid or voluntary primary caregiver 3) home environment supports home care services.
Continually reassess resource utilization as patient progresses towards goals. Reviews, prioritizes and implements POC based on payer limitations.
Completes assignment by managing all anticipated patients care needs during visit and through next planned nurse visit. Communicates to Case Manager and others on care team any changes in Plan of Care.
Organizing/Staffing/Directing: Coordination of services:
Communicates consistently and effectively with patient, caregiver, significant others, physicians and all team members to assure appropriate implementation and completion of the patient plan of care.
Manages patient expectations.
Advances the POC with the interdisciplinary team. Consults with the other team members as needed to make modifications in the plan. Decreases utilization of services as patient moves towards goal achievement.
Supervises paraprofessional staff in patient care.
Attends and participates in patient care rounds.
Communicates/coordinates with Clinical Resource, Utilization Review and/or MA Specialists.
Control: Maintains complete, accurate, legible and timely clinical records/ documentation on all patients according to agency policy.
Completes majority of documentation at the point of care.
Meets all compliance measures (ex: MD orders, insurance authorization, patient rights). Timely submission of all documentation.
Regular communication with manager regarding concerns and problem solving.
Fiscal Accountability: 1) is a good steward of resources: discipline utilization, visits and supplies within payer limits 2) maintains productivity standards.
Professional Intervention: Provides safe, effective skilled and advanced intervention, within scope of practice according to patient need.
Provides counseling and teaching of patient, family and caregiver regarding care needs and health promotion/prevention.
Evaluates and documents response to care/services delivered.
Professional Development: 1) stays current (expert) in home care regulation and all applicable licensing, 2) actively participates in educational programs, meetings and agency committees as appropriate, 3) accepts responsibility for the understanding and implementation of the discipline applicable content of all departmental communications and procedures, 4) identifies processes to improve and potential solutions.
One year recent RN experience
Ability to function in job role independently
Computer skills: able to document effectively in EMR
Licensed as a RN and eligible to practice in Minnesota
Current CPR certification (BLS for Healthcare Providers)
Bachelors degree in nursing
Two or more years RN experience in oncology, med/surg, critical care or high tech home care/ hospice.
Public Health Nurse (PHN) certification
