Drive to locations (homes/facilities) primaily in the Eastern Metro Cities/Suburbs in Minnesota
Monday-Friday, with weekend rotation (typically every 4th), Day shifts, .8FTE
Home office location is in Maplewood MN off beam Ave
Maintains knowledge of, and complies with, all relevant laws, regulations and policies, procedures and standards.
Actively participates in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.
Delivers effective and appropriate home nursing care, establishing key relationships with patients and family members for the purpose of advocacy and coordination of care.
Collaborates with RN Case Manager and other health professionals as appropriate in determining patient care goals.
Educates the patient/caregiver how to self-manage their disease on a day-to-day basis in collaboration with the RN Case Manager.
Recognizes and reports signs of impending physiologic and/or psychological patient changes to the appropriate physician and RN Case Manager to implement appropriate patient care interventions.
Recognizes, evaluates and documents needs of patient and family to provide the most effective and appropriate patient home health care.
Maintains complete, accurate, legible and timely clinical records/ documentation on all patients according to agency standards and/or policy.
May direct work of LPN's or home health aides
Associate's degree in Nursing required
Must have one or more of the following: One year recent RN experience, or One year LPN experience in Fairview Home Care and Hospice or One year TCU or SNF experience
MN RN license
BLS for health care providers
DMV check, current driver's license, reliable transportation and automobile insurance
Computer skills: able to document effectively in Electronic Medical Record (EMR)
Previous RN experience in high-tech home care or hospice, med/sug, oncology, or critical care
Bachelor's degree in Nursing or related healthcare area
