Performs skilled nursing services to home health and hospice patients in their home throughout Dare County, NC. Acts as case manager, performs assessments and delivers direct care to a group of home bound clients. Works as a member of a team of professionals including medical director, nurses, therapists and certified nursing assistants. The work week is 37.5 hours. Shares on call responsibilities. The successful candidate will be employed by the County of Dare and the position includes a compliments of benefits.
ABOUT THE POSITION:
Performs intermediate skilled human support work performing nursing services for a variety of County programs, and related work as apparent or assigned. Work is performed under the limited supervision of the direct supervisor. Continuous oversight is exercised over certified nursing assistant.
Completes skilled nursing assessments and home safety assessments. Refers patients to community resources or other disciplines as needed. Teaches family and patient how to administer medications. Completes required documentation of all visits and communications. Performs Case Management. Assists with state/agency audits. Health: Conducts visits with Home Health and Hospice patients and families; confirms patient has appropriate medications and equipment; obtains any needed orders. Conducts visits and completes assessments as needed, Obtains ordered labs. Completes wound care.
Thorough knowledge of the principles, standards, practices and techniques of public health programs; thorough knowledge of the diagnostic, therapeutic and medical interventions related to personal health; thorough knowledge of public health nursing, education, nutrition, counseling, laboratory and pharmacy services; ability to exercise sound judgment in appraising situations; ability to plan and supervise the work of subordinate staff; ability to maintain accurate medical records; ability to establish and maintain effective working relationships with administrative staff, medical staff, associates and the general public. .
EDUCATION, EXPERIENCE AND SUPPLEMENTAL INFORMATION:
Associate's degree in nursing and moderate experience in nursing and case management, or equivalent combination of education and experience. Special Requirements CPR Certification Possession of unencumbered NC Registered Nursing License upon hire Valid driver's license in the State of North Carolina
