Recruitment/Relocation Incentives: NOT Authorized The New Mexico VA is seeking a Licensed Practical Nurse for the Community Based Outpatient Clinic in Raton, New Mexico! As a Licensed Practical Nurse (Telehealth), you will serve as a coordinator of telehealth services in the Community Based Outpatient Clinic in Raton, New Mexico. You will be responsible for the patient site clinical, business, scheduling and technical aspects for real-time telehealth applications, to include but not limited to tele-mental health, tele-audiology, Home Based Primary Care, Pre/Post op surgery; neurology, cardiology, tele-rehabilitation, telehealth imaging, and tele-patient education clinics. You will develop programs and processes to ensure telehealth modalities are timely, efficient and appropriate to the needs of the patient such as; scheduling, confirming and coordinating telehealth services of in/out patients from VA, other Federal, and private facilities including nursing homes, hospitals and clinics. You will serve as a member of the multi-disciplinary Patient Aligned Care Team (PACT); providing accessible, coordinated, comprehensive, patient centered care. Direct patient care could include but is not inclusive of performing temperature, blood pressure, respiratory rate, oxygen saturation measurements, measuring height and weight, and patient transfers in support of the primary care or specialty clinics.
