The Charles George VA Medical Center's Nursing Service has one (1) vacancy for a Licensed Practical Nurse (LPN) with the Community Living Center at our facility providing care to a variety of patients with varying diagnoses. PLEASE NOTE: First area of consideration given to current, permanent Charles George VA Medical Center. Work Schedule: 7:30 am to 8:00 p.m.(6 days); 7:30 a.m. to 4:00 pm (1 day) - per two week pay period. Financial Disclosure Report: Not required Duties of this position include but are not limited to the following: Collects data and collaborates in the assessment of the patients health status. Initiates appropriate emergency interventions. This position requires maintenance of BLS and ACLS certification. Provides direct nursing care to patients/residents in a long term care setting. Recognizes population-specific patient needs and applies knowledge in delivering care. Demonstrates ability to serve as a responsible member of the health care team and interact with patients, family members and co-workers Demonstrates high level of customer service and is courteous with each interaction. Documents care provided, observations, and responses on the electronic medical record, tools and/or forms as appropriate. Observes and reports changes in patient's/resident's condition, (e.g. behavior shifts, mood changes, skin coloring, appetite, etc.) Assures receipt of adequate nutritional intake through observation, feeding, or properly positioning and checking placement of feeding tube for the patient/resident. Monitors intake and output, according to established policy and procedure. Assists with recreational and socialization activities. Anticipates a clean, safe and therapeutic environment and takes appropriate actions. Uses proper body mechanics. Appropriately uses lifting/transport equipment, protective devices and staff assistance as indicated according to plan of care. Identifies and reports potentially hazardous environmental factors and initiates appropriate actions to prevent injury. Recognizes the patient's/resident's autonomy independence by means of respect, limits setting, and individualized treatment Provides effective education to patients/residents in relations to common disease processes, medications, and/or prescribed treatment regimes Integrate and apply the National Patient Safety Goals as applicable to their area of practice Maintains confidentiality of written, electronic, and verbal patient/employee information Integrates and applies pertinent aspects of the Long Term Care performance improvement program into his/her professional practice when appropriate.
