This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The incumbent in this position functions as a Case Manager for St. Louis VA Health Care System's HUD-VASH Program. The HUD-VASH Program falls within the Healthcare for Homeless Veterans (HCHV) Programs of the Mental Health Service. The incumbent is responsible for coordination of intensive, recovery oriented case management services for Veterans receiving housing provided through HUD Section 8 housing vouchers. The functions include, but not limited to: Responsible for providing case management for a caseload of Veterans in the HUD-VASH program. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placement. Performs home visits for HUD-VASH participants to ensure that Veterans reside in safe environments in compliance with local housing codes. Responsible for providing HUD-VASH case management. Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Incumbent takes responsibility for improving professional skills, participating in planning and executing specific staff development projects. The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager. The incumbent performs accountability functions: 1) documents all contact with patients/families in the medical charts: 2) completes statistical and other reports (including workload reports) in accordance with VA Health Care System and respective professional discipline service policies. The incumbent works with VA Health Care System multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program Veterans.
