The Purchased Geriatrics and Extended Care (PGEC) Program is located within the Home and Community Based Services (HCBS) Department of the Rehabilitation and Long Term Care (RLTC) Division at VA Portland Health Care System (VAPORHCS), on the Vancouver campus. The Community Health Coordinator (CHC) for the PGEC Program assesses and coordinates patient care through the following activities: collaborating/consulting with interdisciplinary team members regarding discharge planning issues, acting as a resource/liaison for exploring appropriate extended care options, providing education regarding referral processes, and facilitating communication between health care providers and community resources. The CHC also functions as approval agent for VA funding of home health, hospice, and home infusion services. In addition, the CHC assists community health agencies in obtaining services, equipment, and medical consultation for Veteran patients. The incumbent is responsible for program development and oversight, including ensuring that agencies meet established criteria for inclusion in the program and that Veterans receive safe and adequate care in their homes. The CHC will complete and document investigations of any complaints, as well as adverse and/or sentinel events involving a purchased skilled home care agency.
