The incumbent works in close collaboration with the Primary Care Medicine Service and any PACT-oversight Committee to meet the requirements of the program including but not limited to scheduling, staffing, referral criteria, and transfer arrangements. These positions are located at either the Honor Annex Community Based Outpatient Clinic or Linwood Community Based Outpatient Clinic or Linwood. Clinical duties include: Assuming primary medical responsibility for their assigned panel of patients who will be primarily older adults with cognitive, functional and complex medical and/or social problems; identifying the patients' medical problems; defining the medical management of these problems; determining the need for consultation from medical, surgical, and/or psychiatric subspecialty clinics and other services, facilitating admission to the hospital when needed; and, when appropriate, serving as collaborator for advanced practice nurses, and providing medical administrative support such as signing home health forms requiring physician signature for the GPC patients who are assigned to NP Providers. May be responsible for providing cross coverage during the absence of physicians in Home Based Primary Care and/or Geriatrics consultation clinic. Administrative duties include: Planning and directing the educational and clinical experience of medical students, residents, and fellows and other learners assigned to the GPC program; assuming a leadership role in the development and implementation of GPC's performance improvement plan; providing clinical input and oversight for patient treatment plans; Responsible for meeting performance measures pertinent to Geri PACT care.
