The Cincinnati OH VA Medical Center is seeking well-qualified candidates for a Registered Nurse to work in the Home Based Primary Care Program at the Bellevue, KY CBOC. This staff nurse will provide intensive care management for patients and assumes accountability and responsibility for his/her nursing practice and decisions affecting patient care. The Registered Staff Nurse in the Home Based Primary Care Program, inclusive of Home Based Primary Care (HBPC), Home Based Transitional Care (HBTC) provides direct nursing care in the home setting . They are responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The Staff Nurse functions as a case manager coordinating the patient care with interdisciplinary team members, interacting, coordinating and advocating for the patient/caregiver with other relevant parties across a continuum of care, beyond a single episode of care, ensuring seamless transitions. The Registered Staff Nurse will assess the status of Home Based Primary Care patients admitted to acute care and rehab facilities via outside facility visits, home visits, telehealth visits, and telephone visits and develops and coordinates the patient's plan of care to include: Ongoing assessment and re-assessment Care planning and coordination Monitoring and evaluating clinical outcomes. The Registered Staff Nurse provides individualized education and support to the patient and caregiver to optimize clinical outcomes and support quality of life decisions. Responsibilities include: Post-acute care case management In-home physical assessments, inclusive of, but not limited to: Medication management Pint of care testing Phlebotomy Wound care Catheter care in the home per established policies, protocols, and guidelines The Registered Staff Nurse provides clinical oversight for a License Practical Nurse, inclusive of and not limited to the following: in home physical assessments, medication management, point of care testing, phlebotomy, wound care and catheter care in the home. The HBPC and HBTC nurse is under the supervision of the Nurse Manger, Home Based Primary Care Programs. The HBTC Registered Staff Nurse provides coverage for the HBPC teams based on patient & staffing needs.
