This provider will provide coverage for PACT teams in the absence of the PACT team provider, either from planned or unplanned leave. This provider may provide coverage at Salem VAMC clinic and/or CBOC sites as needed. The GAP PACT Provider will is central member and leader of the Primary Care PACT team. He/she is responsible for the assessment and management of a panel of approximately 1200 Veterans. The PACT Physician works in collaboration with a multi-disciplinary team including RN, LPN, Clinical Pharmacy Specialist, Dietician, Social Workers, Mental Health Professionals, etc. to provide comprehensive primary care services. FUNCTIONS: Clinical: Primary Care: Approximately 70%-90% time Administrative Support: 10% Resident/Medical Student Teaching: 0-30% time if applicable Pay: Competitive salary, annual performance bonus, regular salary increases.
