This announcement will be used to fill multiple positions within the HUD/VASH Program. This position is assigned to the Captain James A Lovell Federal Health Care Center (FHCC) providing services to Veterans, Active Duty Service Members, Retirees and their Dependents. FHCC is a partnership between the Department of Veterans Affairs and the Department of Defense (DOD), integrating all medical care into a fully integrated federal health care facility with a single combined VA and Navy mission. The duties listed reflect those performed at the full performance level. Candidates selected at lower grade levels will serve in a developmental capacity. The incumbent is responsible for monitoring all phases of programming and coordination of case management services for veterans receiving housing provided through HUD Section 8 housing vouchers. The initiative is known as the HUD-VASH program and was authorized pursuant to Division K, Title II, of The Consolidated Appropriations Act, 2008 (Pub. Law 110-161) ("2008 Appropriation Act") enacted on December 26, 2007 (see proviso (7) under the heading "Tenant- Based Rental Assistance"). The HUD-VASH program combines HUD HCV rental assistance for homeless Veterans with case management and clinical services provided by the Department of Veterans Affairs (VA) at its medical centers and in the community. Responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. Performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes. Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation. Responsible for providing oversight of HUD-VASH case management. Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the HUD-VASH Case Manager. Participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH. Responsible for administering local policy and SOP for the HUD-VASH Program. Participates in the orientation/teaching of graduate social work/nursing students, students of other disciplines, and Medical Center personnel on the biopsychosocial aspects of homelessness. Works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program veterans. Conducts psychosocial assessments to determine needs of veterans and family members. Participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. Provides individual, family and group counseling as appropriate and as scope of practice and licensing allows. Provides consultation/education to veterans and families on community resources, advance directives and VA benefits. Acts as liaison between VA and community resources, to include marketing and public relation duties as needed. Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
