The incumbent assigned to the Urgent Care Service. The Nurse Practitioner (NP) is a registered nurse with advanced education and experience who applies specialized clinical medical and nursing knowledge and skills to patient care and in the improvement of the quality of care provided by others. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Must possess a Master's Degree in Nursing with an emphasis as a Nurse Practitioner from a school of professional nursing that was accredited by the State agency, and accredited by the Accreditation Commission for Education in Nursing (ACEN; http://www.acenursing.org) formally, the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE; http://www.aacn.nche.edu) at the time you completed the program Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Must possess a current, full, active and unrestricted certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse II - A Master's degree in nursing and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Moderate lifting 15-44 pounds; Light carrying under 15 pounds; Straight pulling 0.5 hours; Reaching above shoulder; Use of fingers; Both hands required; Walking 8 hours; Standing 8 hours; Repeated bending 5 hours; Both legs required; Near vision correctable at 13" to 16" to Jaeger 1 to 4; Far vision correctable in one eye to 20/20 and to 20/40 in the other; Both eyes required; Depth perception; Ability to distinguish basic colors and shades of colors; Hearing (aid may be permitted); Emotional and mental stability, Typing and computer work 6 hours per week; Basic life support skills including chest compressions. Environmental Factors: Working closely with others; Working alone; ["Specific activities will include (but are not limited to): - Serve as reference to Patient Aligned Care Teams (PACT) / specialty teams to facilitate timely, appropriate work-up in collaboration with specialist(s) optimizing VA resources and facilitating community care as needed\n- Point of contact for any work-up related questions from patient/family, staff (local and/or other Veterans Integrated Services Networks (VISN)20 facility), community providers\n- Inform patient/family of process, general disease education\n- Assure proper follow-up of abnormal findings that are not treated during visit/stay.\n- Identify high risk patients by acuity and/or biopsychosocial issues for intensive case management\n- Monitor activities to ensure timely multidisciplinary review and coordination of activities necessary for development of a treatment plan and necessary supportive procedures\n- Assess and link patient to additional services when needed, i.e. alcohol treatment, nutrition, palliative care, hospice as necessary, Home Telehealth\n- Communicate/coordinate with PACT team regarding medications, patient care needs, etc.\n- Support patient education, treatment education, awareness of steps in care\n- Assess and treat patient for symptom management and/or refer to appropriate specialty for higher level care needs\n- Track all treatments according to care plan, communicating with appropriate service provider on any deviations/omissions from plan\n- Assure treatment summary and recommendations are communicated to involved services\n- Proactively addresses discharge/continuity of care needs (home care, supplies, medications, etc.).\n- Works collaboratively with Patient Flow and the Centralized Consult Center for internal and external referrals\n- Provides education to patient/family for medication changes, symptom management, etc.\n- Ensure all mandatory reporting, documentation, coding, and encounters are completed timely and tracked, as appropriate\n- Serves as a point of contact for any medicine-related questions from patient/family, staff (local and/or other VISN20 facility), and community providers - Facilitate transition of care for ongoing needs\n- Ensures provision of biopsychosocial support to patient/family\n- Participate in quantitative and qualitative data collection to support evaluation efforts of the unit\n- Work directly with stakeholders to identify needs, systems issues, and communicate activities, treatments, and ongoing needs\n- Partner with local Leadership in communicating and addressing systems issues identified by VISN, or regulatory agencies for timely resolution\n- Partner with the Leadership on regional and VISN-wide process improvement strategies - Serves on local and VISN committees Work Schedule: Variable Shifts; UCC hours are 8:00am - 8:00pm;\nTelework: Not available;\nVirtual: This is not a virtual position.\nRelocation/Recruitment Incentives: Not authorized;\nPermanent Change of Station (PCS): Not authorized;\nFinancial Disclosure Report: Not required"]
