Chilton Medical Center, part of Atlantic Health System is seeking a Part Time Social Worker ( MSW ) for their Case Management dept.
Part Time, Day time shift with weekends and holidays on rotation.
Provide social work coverage to facilitate discharges.
Meet with patients and families to discuss and support the plan of care and engage them in discharge planning.
Provide resources, referrals and support as needed on a case by case basis.
Coordinator transitions of care to extended care facilities, inpatients behavioral health and chemical dependency facilities, hospice and palliative care programs, LTAC and Rehab.
Attend daily huddles and interdisciplinary rounds and work closely with the healthcare team to implement the plans of care.
Obtain all necessary third party authorizations, communicate with community resources and area facilities.
Daily use of phone, fax and electronica documentation in EPIC.
MSW with current NJ State Social Work license. LSW or LCSW.
Qualification / Requirements
Organization, ability to work simultaneously on more than one case.
Time management a must.
Phone, fax and computer skills, willingness to work with a team.
Compassionate service delivery.
Ability to work with all age groups.
Hospital experience and documentation in EPIC experience preferred.
