Makes referrals to approach social service agencies and arrange for placement as directed. Consults with MSW SW as needed for clinically complex patients or difficult family dynamics. What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:   Bachelors Degree (Social Work, Professional Counseling, Marriage, Famly and Child counseling)1-3 years of experience in Social Work with emphasis on discharge planning, referral to community resources and/or case management.
