Overview
Atlantic Health System is seeking an LPN Clinical Specialist to work Monday - Friday 9am to 5pm located at Basking Ridge, NJ.
Responsibilities
Essential Functions/Responsibilities of Position:
Provide Clinical support services to APCS clients and case managers.
Monitor the policies and procedures necessary to maintain the standards required by accrediting bodies and/or licensing agencies.
Communicate with clients and families.
Complete incident forms.
Supervise the follow-through with MD orders, insurance authorizations/approvals, care plans, medication profiles, assessments, reassessments, admission paperwork, discharge paperwork, case conferences, and departmental / staff meetings.
Visit private duty nursing cases at the request of the clinical director.
Train agency staff on software computer programs and materials.
Assist in training new field case managers.
Act as clinical support resource to field staff nurses and visit as necessary and through on call in accordance with the on-call schedule.
Supervise CHHA team members complete annual evaluations.
Monitor CHHA performance.
Conduct Quality assurance audits.
Follow up and document on staff/ client complaints and concerns.
Provide input into the development of the annual in-service program.
Conduct in services throughout the year.
Assist Nursing Coordinator with private duty nursing (PDN) cases, as necessary.
Assist Scheduling Coordinators and HR Manager, with coordinating supervisions and client monitoring visits.
Insure case manager visits are completed.
Screen prospective Skilled Nursing clients.
Conduct Chart audits as assigned.
Qualifications
Minimum Education:
Associates Degree in Nursing
LPN License in NJ
Minimum Experience:
At least 2 years experience in a home health care setting or revenant clinical setting
Experience working with home health care standards and quality assurance / risk management
One year acute care nursing
